UHC Voluntary Enrollment
Voluntary students are now able to enroll in the Student Health Insurance plan by going to and searching in the text box for 食色视频. Once on the schools UHCSR homepage, they will find the voluntary enrollment tile and on that page there will be a 鈥淕et Started鈥� link. From there the student will fill out the required information for enrollment through the website.
UHCSR reduced the creation of paper enrollment forms in an effort to become more green. Students should use the UHCSR site for voluntary enrollment. The deadline to register for the voluntary plan for Fall 2024 will be available by September 8, 2024.

UHC Qualifying Life Event Form
Use the below form if you have experienced a Qualifying Life Event (QLE) and wish to enroll in the SHIP plan for the remainder of the current coverage period. Examples of QLEs are a change in marital status, birth of a child, job loss or loss of coverage due to age.
